Life can be unpredictable, and we understand that plans may change.
If you need to reschedule or cancel your appointment, we kindly request a minimum of 24 hours' notice. This allows us to accommodate other patients.
Please note that appointments cancelled within 24 hours within any two-month period may be subject to a cancellation fee.
Under data protection legislation an individual has the right (subject to certain exemptions) to access the information that an organisation holds about them.
Accessing personal data in this way is known as making a subject access request. If you would like to access your personal data you can complete a Subject Access Request form which can be located on our Subject Access Request page
